What is Degenerative Disc Disease?

If you have chronic issues with your back, you may have heard about degenerative disc disease, but not know much about it. Let’s take a look at the basics.

The Short Description
Degenerative disc disease is often associated with older individuals, even though younger people can have it as well. The intervertebral discs are little cushions between the vertebrae in the spine. They both allow your back to perform complex motions and maintain stability, all while allowing your back to carry the weight of your body and more. As people age, the discs can lose elasticity and become thinner, even losing some of the gel-like fluid inside the discs.

When this occurs, you lose flexibility in motion and you can experience pain with increasing intensity and frequency. If you have chronic neck or back pain that seems to occur more often and be more painful and/or are experiencing a loss of back flexibility, it can be a sign that you have degenerative disc disease.

Where is the Pain?
The pain for degenerative disc disease most frequently occurs in the lower back area, known as the lumbar spine. It is also commonly seen in the neck area, known as the cervical spine.

Understanding Your Spine
In your backbone, also known as the vertebral column, there are 33 vertebrae. Between your vertebrae, there are intervertebral discs, as described above, that act like tiny pillows between your vertebral bones. Each disc consists of an outer band (annulus fibrosus) and an inner liquid substance (nucleus pulposus). The vertebrae combined with the discs, form a protective exterior for the spinal cord and nerves. 

On the back side of your vertebrae are facet joints which also assist with movement and flexibility. All of the spinal joints are protected by cartilage, which can also wear away and will compound the pain experienced. Additionally, the back has muscles, ligaments, and tendons. Muscles power the movement that takes place. Ligaments link your bones together while tendons connect the bones to muscles.

Degenerative Disc Disease
Degenerative disc disease alters the discs, making them less capable of cushioning your back’s motions. Your discs may also bulge or even herniate. Degenerative disc disease is not really a disease as much as it is part of the normal wear and tear of aging.  Gradually the outer part of the discs (the annulus fibrosus) weaken and the water content of the discs decrease, making your discs more stiff and rigid.

Causes of Degenerative Disc Disease
Because degenerative disc disease is usually the result of aging, people over 50 are more likely to experience this issue. Injuries can begin the degenerative process in the discs in your back as well, so younger people can also, though rarely, experience degenerative disc disease as a result.

Does Degenerative Disc Disease Require Surgery?
Some patients may require surgery, but the large majority of patients can find relief without surgery. For those who need surgery, there are several common methods used to help patients with degenerative disc disease: Foraminotomy, Macrodiscectomy, Anterior cervical discectomy and fusion (ACDF).

How to Tell If You Have Sciatica

Many people think that sciatica is back pain that radiates down your legs, accompanied by a quick jolt. Contrary to popular belief, sciatica doesn’t usually involve much back pain at all. Sciatica is really a symptom of a pinched nerve and involves shooting leg pain that can be found anywhere from the hip area down to the toes.

Here are some additional ways to tell if your pain is sciatica.

Feeling Leg or Knee Weakness
The sciatic nerve is the largest single nerve in the human body, stretching from the lower spine to the foot. When this nerve is pinched, its ability to function is interrupted and results in painful sensations, tingling, and weakness. While pain is what will cause patients to seek care, the combination of pain and weakness in the same leg is a red flag that the sciatic nerve is involved.

You’re Not a Runner
Runners will frequently experience all the same symptoms of sciatica, but it can turn out to be piriformis syndrome. A slipped disc still causes similar symptoms but it presses on the piriformis muscle. If you are not a runner, then it is likely your issues are sciatica.

You Can’t Pinpoint the Exact Source of Your Pain
Use your thumb to press on various areas in your lower back and legs. If you can locate a spot that seems to be the true location of your pain, then your issue is muscle related. Muscles, when injured, can radiate pain throughout the body, particularly if the muscle is shortened (contracted) and tight. If you cannot find a specific source for your pain, then your sciatic nerve is a likely source.

You Can’t Pass the Test
Doctors use the following simple test to determine if someone has sciatica. Start by lying down and stretching out your legs. Have someone help you raise your leg to between 30 and 70 degrees while keeping your legs straight. If you experience pain—especially pain that radiates down to below your knees and even to your toes, you likely have sciatica. This test stretches the sciatic nerve which if it is compressed or injured will produce pain during this motion.

Extra Trips to the Bathroom
This is a sign that your sciatica is much more serious, especially if you have the typical sciatic pain and totally lose control of your bowel and or bladder movements. What Do Studies Show About Chiropractic Care

Journal of Manipulative and Physiological Therapeutics-Childhood Asthma
The Journal reports the use of manual techniques on children with respiratory diseases can be beneficial. Chiropractic and massage are two of the most commonly used interventions.

The European Spine Journal-Sciatica
Findings from a clinical trial were published in The European Spine Journal last year. The results of the trial showed that chiropractic adjustments resulted in a 72 percent success rate in treating sciatica and related symptoms compared to the 20 percent success rate from physical therapy and 50 percent from corticosteroid injections.

British Medical Journal 2003-Low Back Pain and Neck Pain
In a study published in the 2003 British Medical Journal, a 52-week study randomly divided 183 patients with neck pain to manual therapy (spinal mobilization), physiotherapy (mainly exercise) or general practitioner care (counseling, education, and drugs). This study showed that chiropractic adjustments helped people recover faster than general practitioner care and physiotherapy. In addition, it showed the total costs of the chiropractic-treated patients were about one-third of the costs of both physiotherapy and general practitioner care.

Journal of Manipulative and Physiological Therapeutics-Low Back Pain
Another study found chronic low back pain patients treated with chiropractic care showed more improvement and satisfaction at the end of one month than similar patients treated by family doctors. The study was published in the Journal of Manipulative and Physiological Therapeutics and resulted a bigger proportion of chiropractic patients (56 percent compared to 13 percent) reporting their low-back pain was better or much better, but on the other hand, nearly one-third of the medical patients reported their low-back pain was worse or much worse.

NIH’s National Center for Complementary and Alternative Medicine-Neck Pain
A study sponsored by NIH’s National Center for Complementary and Alternative Medicine researched the effectiveness of different approaches for treating neck pain. There were 272 participants who were divided into three groups that received either pain medication (over-the-counter pain relievers, narcotics and muscle relaxants), spinal manipulative therapy from a doctor of chiropractic (DC), or exercise recommendations. After 12 weeks, about 57 percent of those who met with DCs and 48 percent who exercised reported at least a 75 percent reduction in pain, compared to 33 percent of the people in the medication group. In a one year, almost 53 percent of the people who didn’t take medication continued to report at least a 75 percent pain reduction, compared to just 38 percent reduction in pain among the people who took the medication.

Journal of the American Medical Association-Neck Pain
A 2008 issue of the Journal of the American Medical Association (JAMA) showed that during a study between 1997 and 2005, the costs for the standard medical management of spinal pain syndromes, both neck and back, increased 63 percent, outcomes for physical functioning, work or school limitations and social limitations for adults declined, and in the number of people who experience neck or back pain increased. However, it also showed that when chiropractic care was included in the managed care program it resulted in cost savings, and a decrease in surgeries, advanced imaging, inpatient care, and plain-film radiographs.

Group Trial-Headaches (Tension and Migraine)
Headaches are one of the most common conditions managed by chiropractors to help cure, prevent and ease the burden of headaches and migraines. A group trial found 49 percent of the people said they had a significant reduction in pain intensity and 22 percent of people who had chiropractic treatment saw the number of attacks drop by 90 percent.

Journal of Manipulative Physiological Therapeutics 2012-Ear Infections and Colic
The Journal of Manipulative Physiological Therapeutics described on a 2012 study showing chiropractic adjustments greatly reduced symptoms of colic. Many case studies have proven that kids with ear infections, acid reflux, and colic can significantly improve after a few visits to a chiropractor.

Case Studies-Scoliosis
Chiropractic adjustments and muscular rehabilitation often help prevent the progression of scoliosis. Case studies in the past have shown that chiropractic care can treat scoliosis without the use of braces and surgeries. It’s been reported to decrease scoliosis curvatures by 10 to 30 percent in some participants.

Human Journal of Hypertension-Blood Pressure
The Human Journal of Hypertension published a 2007 study showing that one upper cervical chiropractic adjustment had the same effect as two blood pressure-lowering drugs. It also stated the effects of just one adjustment lasted more than six months.

Here are some suggestions to help get you back in the after-holidays swing of things again.

Just Get Started

The most important thing is to just start. The holidays are over. You can’t take it back, but you can make the most of the present. Today’s a new day. Make a plan, get started, and stay committed. 2017 is your year to be your healthiest self.

If you want to change things up, find an activity that you enjoy and can give you a good workout. Go slow if needed so that you don’t hurt yourself. No matter who you are, exercise is important to keep your body healthy. You will feel better and have more energy the more fit you get. It’s not easy to do. You sometimes have to push yourself to keep going, but it’s worth it in the end.

Schedule Time

Workout Regularly

Getting back into your workout routine can be jumpstarted by challenging yourself or your workout partner. Commit to your workout for 30 days and set a pattern for your workouts. At the end of your challenge period, you will have developed the habit of working out again and you will continue it.

Start Slowly

Once you have a schedule, keep it by starting out with shorter workouts. If you can’t do the entire workout, do something.
